ADMISSIONS
Applications are welcomed on recommended Admission Types, from applicants having Home (No other certification is acceptable) of any region of Punjab or Government Investment Place (Islamabad) and who have showed up in the Entry Analyze organised on Twenty third Sept, 2012, for entrance to Govt Healthcare and Oral Organizations of the Punjab
WHO CAN APPLY
For entrance against Start Benefit chairs, only those applicants having an combination ranking of 80% or above marks(excluding Huffaz-e-Quran) can apply
According to PMDC Guidelines, combination represents shall be measured by including the represents as follows:
Matric or Comparative ------ 10%
FSc or Comparative ------ 40%
UHS Entry Analyze ------ 50%
The system for computation of Aggregate Marks is also available on UHS web page www.uhs.edu.pk
The above mentioned situation is not appropriate to applicants looking for entrance against any type of arranged chairs (Disabled Learners, Under-Developed Regions, Cholistan) who can implement if they meet primary qualifications requirements as set down in the Prospectus and also available on UHS web page www.uhs.edu.pk.
There are 09 chairs available on Shared foundation for Punjab domiciled applicants in the medical colleges of Balochistan, KPK and Azad Jammu & Kashmir. Candidates implementing against open merit chairs can work out their choice for these chairs as well. No individual program will be welcomed for these seats
HOW TO APPLY
The Admission Packages containing Prospectus and Admission Type, will be available, without charge, at School of Wellness Sciences, Khayaban-e-Jamia Punjab, Lahore, Rawalpindi Healthcare Higher education Rawalpindi and Nishtar Healthcare Higher education Multan, from Thirty first Oct to Tenth Nov, 2012 (excluding Weekend and Iqbal Day), from 09:00 am to 03:00 pm. The applicants will have to offer the attested duplicates of their Matriculation, FSc (or Equivalent) and Entry Analyze Outcome Cards and domicile certification to be able to get the Admission Kit.
Candidates must publish the usually loaded Admission Types alongwith all needed records as offered in the Prospectus, IN PERSON in the middle from where they obtained their kit, from Thirty first Oct to Tenth Nov, 2012 (excluding Weekend and Iqbal Day), from 09:00 am to 03:00 pm. Admission forms obtained by publish shall not be entertained
Candidates will provide their choice for medical and dental colleges in one individual entrance form which once given shall be last and cannot be modified consequently. An candidate shall not be regarded for an excellent he/she has not known as in his/her purchase of choice. One candidate can only publish one form. There is no need to publish individual forms for MBBS and BDS
Forms once posted shall not be came back. Imperfect or unsigned entrance forms will be rejected

IMPORTANT
No entrance form will be approved after 03:00 pm on Sunday, Tenth Nov, 2012.
Once the doorway form is posted, no other papers (e.g., improved represents certification, domicile certification, etc) shall be appropriate. The records posted with the program shall be regarded as last.
Candidates having international credentials such as A/O-Level etc., shall have to offer the Equivalence Certificate from IBCC.
Any candidate discovered to have posted made accreditations or bogus records with the doorway form shall be completely debarred from entrance to any medical/dental college.
Candidates and their mother and father are recommended to properly go through the Prospectus before stuffing up the Admission Form.
